Неоадъювантная химиолучевая терапия позволяет уменьшить объем опухоли, добиться полного клинического ответа в трети случаев, что приводит не только к увеличению числа сфинктеросохраняющих операций, но и повышает общую и безрецидивную выживаемость. В качестве химиотерапевтических агентов, являющихся радиомодификаторами и за счет этого повышающих эффект лучевой терапии, применяются препараты фторпиримидинового ряда (5-фторурацил, капецитабин). Применение их может сопровождаться побочными эффектами, что нередко приводит к коррекции плана лечения или невозможности проведения его в полном объеме. Поиск новых подходов для усиления эффекта лучевой терапии путем повышения концентрации химиотерапевтического препарата в опухоли и уменьшения при этом системной токсичности является актуальной задачей в настоящее время. В работе предлагается способ неоадъювантной химиолучевой терапии с использованием интраректального стерильного гидрогелевого материала на основе альгината натрия, содержащего инкорпорированный 5-фторурацил.</p></abstract><trans-abstract xml:lang="en"><p>Chemoradiotherapy, along with following surgical treatment, including total mesorectal excision, is a current standard for treatment of locally advanced rectal cancer. Neoadjuvant chemoradiotherapy allows to downsize the tumor, up to complete clinical response in one third of all cases, which, in turn, allows increase in sphincter preserving operations, disease-free period and overall survivability. Derivatives of 5-fluoracil are used as basic chemotherapeutic agents. These drugs have a substantial amount of side effects, which lead to either plan corrections, or may even prevent its completion. Thus, search for new ways to increase chemotherapy agent in the tumor cells with reduction of systemic toxicity to improve neoadjuvant chemoradiotherapy results is an acute task for modern medicine.
