Охарактеризовать негативное воздействие механизмов инсулинорезистентности на измененную вследствие цирроза печеночную паренхиму можно с помощью КТ-перфузии печени – метода, позволяющего оценить гемодинамические нарушения в ткани печени. В ходе пилотного исследования было установлено, что значения артериальной перфузии и индекса печеночной перфузии у пациентов с компенсированным циррозом печени при наличии сахарного диабета 2-го типа были статистически значимо выше, чем при отсутствии данной патологии (р = 0,007 и р = 0,009 соответственно). Значения портальной и общей перфузии у пациентов с компенсированным циррозом печени при наличии сахарного диабета 2-го типа были статистически значимо ниже, чем при отсутствии данной патологии (р &lt; 0,001 и р = 0,007 соответственно). КТ-перфузия печени может применяться у пациентов с компенсированным циррозом и наличием сахарного диабета 2-го типа в анамнезе в качестве неинвазивного метода инструментальной диагностики изменений гемодинамики, в том числе в оценке прогрессирования портальной гипертензии и обнаружении в последующем сопутствующих осложнений.</p></abstract><trans-abstract xml:lang="en"><p>Currently the relationship between liver diseases and glucose metabolism disorders is being studied. To characterize the negative impact of the mechanisms of insulin resistance on the liver parenchyma altered due to cirrhosis, we can use CT-perfusion of the liver — a method that allows us to assess hemodynamic disorders in the liver tissue. During the pilot study, it was found that the values of arterial perfusion and hepatic perfusion index in patients with compensated cirrhosis of the liver in the presence of type 2 diabetes mellitus were statistically significantly higher than in the absence of this pathology (p = 0,007 and p = 0,009 respectively). The values of portal and total perfusion in patients with compensated cirrhosis of the liver in the presence of type 2 diabetes mellitus were significantly lower than in the absence of this pathology (p &lt; 0,001 and p = 0,007 respectively). CT-perfusion of the liver can be used in patients with compensated cirrhosis and the presence of type 2 diabetes mellitus in the anamnesis as a non-invasive method of instrumental diagnosis of hemodynamic changes, including in assessing the progression of portal hypertension and the subsequent detection of concomitant complications.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>перфузия</kwd><kwd>компьютерная томография</kwd><kwd>цирроз печени</kwd><kwd>диабет</kwd></kwd-group><kwd-group xml:lang="en"><kwd>Perfusion</kwd><kwd>Computed Tomography</kwd><kwd>Liver Cirrhosis</kwd><kwd>Diabetes</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Elkrief L., Rautou P.
