В исследование включен 31 пациент, страдающий метастазами НЭО в печени, которым выполнен 61 сеанс ТАХЭ. Проанализированы данные 108 попарных МРТ-исследований, выполненных до и после процедуры. В солидной части таргетных очагов до и после ТАХЭ количественно оценивали следующие показатели: измеряемый коэффициент диффузии (ИКД) и его неоднородность (стандартное отклонение - СО), интенсивность сигнала (ИС) на Т1-взвешенных изображениях (Т1-ВИ) до и после введения МР-контрастного средства (МРКС) и СО ИС на Т1-ВИ в портовенозную фазу контрастного усиления. В солидной части таргетных очагов после ТАХЭ выявлено повышение ИКД (р = 0,002) и его СО (р = 0,006), уменьшение степени накопления МРКС в артериальную (p = 0,016) и портовенозную (p = 0,004) фазы, увеличение скорости вымывания МРКС (p = 0,016) и СО ИС на Т1-ВИ в портовенозную фазу контрастирования (p = 0,026). Положительными прогностическими факторами, связанными со временем до прогрессирования, оказались повышение ИКД и его СО, снижение накопления МРКС в портовенозную фазу. Использование МРТ-параметров, характеризующих изменение структуры солидного компонента метастазов НЭО в печени после ТАХЭ (ИКД и накопление МРКС), позволяет более точно спрогнозировать время до прогрессирования заболевания и персонализировано подойти к определению интервалов наблюдения за пациентами.</p></abstract><trans-abstract xml:lang="en"><p>Purpose - to identify MRI parameters suitable for assessment of metastatic neuroendocrine tumor (mNET) textural changes after liver transarterial chemoembolization (TACE) and their relationship with the progression-free survival. Thirty one hepatic mNET patients undergone 61 TACE procedures were enrolled in the study. One hundred eight pairwise (pre- and post-TACE) MRI studies were analyzed. Before and after TACE we measured apparent diffusion coefficient (ADC) and its standard deviation (SD), MR contrast agent (MRCA) uptake dynamics and signal intensity (SI) SD on portal phase T1 WI in the tumor solid component. The solid portions of target mNET demonstrate the following MRI changes after TACE: increase of ADC values (р = 0,002) and its SD (р = 0,006), decrease of MRCA accumulation on arterial (p = 0,016) and portal (p = 0,004) phases, increase of both MRCA wash-out time (p = 0,016) and SI SD on portal phase T1-WI (p = 0,026). The increase of ADC values and its SD and decrease in MRCA accumulation on portal phase were significantly associated with increase to progression free survival.
