Билиарные осложнения, такие как стриктуры анастомоза общего желчного протока и биломы, остаются одними из наиболее частых (10–30 %) и серьезных проблем в послеоперационном периоде у реципиентов печени, существенно влияя на приживаемость трансплантата и исход трансплантации в целом. Своевременная диагностика является ключевым звеном успеха в их лечении, однако в настоящее время отсутствует единый протокол лучевого мониторинга пациентов.</p></sec><sec><title>Цель</title><p>Цель. Оценить возможности МСКТ/МРТ в диагностике и прогнозировании билиарных осложнений у пациентов в раннем и позднем периодах после трансплантации печени с учетом разработанного алгоритма лучевого сопровождения.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено одномоментное одноцентровое ретроспективное исследование 67 пациентов, перенесших трансплантацию печени. Всем пациентам выполнялись МСКТ и МРТ/МРХПГ по стандартизированному протоколу в предоперационном периоде и динамически в послеоперационном периоде (на 3–7-е сутки, через 1 и 3 месяца). Пациенты были разделены на группы: с развившимися билиарными осложнениями (n = 18) и без (n = 49).</p></sec><sec><title>Результаты</title><p>Результаты. Применение протокола динамического мониторинга позволило значительно снизить частоту персистирующих билиарных осложнений по сравнению с предполагаемой частотой без его использования (26,9 % vs 44,8 %, p = 0,026), что снижало шансы их развития в 2,25 раза. Раннее выявление позволило успешно купировать осложнения в 64,3–75 % случаев с помощью малоинвазивных вмешательств. Выявлены значимые предикторы риска билиарных осложнений: болезнь Вильсона — Коновалова (ОШ = 4,13) и первичный склерозирующий холангит (ОШ = 9,86).</p></sec><sec><title> Выводы</title><p> Выводы</p></sec><sec><title>1</title><p>1. Этиология исходного заболевания печени является значимым предиктором риска развития билиарных осложнений после трансплантации. Наибольший риск ассоциирован с болезнью Вильсона — Коновалова (11,1 % против 0,0 %, р = 0,017, ОШ = 4,13; 95 % ДИ 2,66–6,41), а также первичным склерозирующим холангитом (16,6 % против 2,1 %, р = 0,023, ОШ = 9,86; 95 % ДИ 1,02–102,42).</p></sec><sec><title>2</title><p>2. Стандартизированный протокол динамического МСКТ/МРТ-мониторинга в раннем и позднем послеоперационных периодах позволяет улучшить диагностику билиарных осложнений (стеноз анастомоза, билома) после трансплантации печени. Ранняя детекция билиарных осложнений позволяет своевременно применить малоинвазивные методы лечения, что приводит к их успешному купированию более чем в 64 % случаев и достоверно снижает совокупную частоту персистирующих билиарных осложнений с 44,8 до 26,9 %, р = 0,026, ОШ = 2,25; 95 % ДИ 1,09–4,63).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Biliary complications, such as choledochojejunal anastomotic strictures and bilomas, remain among the most common (10–30 %) and serious problems in the postoperative period for liver recipients, significantly affecting graft survival and the outcome of transplantation in general. Timely diagnosis is a key factor for successful treatment; however, there is currently no unified protocol for radiological monitoring of patients.</p></sec><sec><title>Objective</title><p>Objective. To assess the diagnostic and prognostic value of a developed standardized protocol for dynamic MSCT/MRI monitoring for the detection and management of biliary complications in patients after liver transplantation.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A single-center retrospective cross-sectional study of 67 patients who underwent liver transplantation was conducted. All patients underwent MSCT and MRI/MRCP according to a standardized protocol in the preoperative period and dynamically in the postoperative period (on days 3–7, at 1 and 3 months). Patients were divided into groups: those with developed biliary complications (n = 18) and those without (n = 49).</p></sec><sec><title>Results</title><p>Results. A standardized protocol for dynamic MSCT/MRI monitoring in the early and late postoperative periods improves the diagnosis of biliary complications after liver transplantation. Early detection of biliary complications enables timely application of minimally invasive treatment methods, leading to successful management in over 64 % of cases and significantly reducing the overall incidence of persistent biliary complications from 44.8 % to 26.9 % (p = 0.026, OR = 2.25; 95 % CI 1.09–4.63). Further studies are needed to validate this method.</p></sec><sec><title>Conclusion</title><p>Conclusion</p></sec><sec><title>1</title><p>1. The etiology of the underlying liver disease is a significant predictor of the risk of developing biliary complications after transplantation. The highest risk is associated with Wilson – Konovalov disease (11,1 % vs 0,0 %, р = 0,017, OR = 4,13; 95 % CI 2,66–6,41) and primary sclerosing cholangitis (16,6 % vs 2,1 %, р = 0,023, OR = 9,86; 95 %CI 1,02–102,42).</p></sec><sec><title>2</title><p>2. The standardized protocol of dynamic MSCT/MRI monitoring in the early and late postoperative periods allows for improved diagnosis of biliary complications (anastomosis stenosis, biloma) af­ter liver transplantation.
