Оценка эффективности выявления стеатоза у пациентов после пересадки печени с помощью магнитно-резонансной томографии с оценкой протонной плотности фракции жира (PDFF).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследовано 113 реципиентов после трансплантации печени в возрасте от 19 до 72 лет (61 женщина и 52 мужчины). Для количественной оценки стеатоза всем пациентам проводилась магнитно-резонансная томография (МРТ) с вычислением фракции жира в печени по протонной плотности (PDFF). Полученные данные по МРТ-PDFF были сопоставлены с результатами биопсии. Расчет процента жировой инфильтрации печени по данным биопсии правой доли печени был использован в качестве референсного метода.</p></sec><sec><title>Результаты</title><p>Результаты. По данным биопсии стеатоз отсутствовал у 81 пациента (72 %), а у 32 (28 %) был выявлен стеатоз различной степени тяжести. Сравнение показало наличие значимых различий показателя PDFF (p ≤ 0,001) у пациентов с наличием и отсутствием стеатоза. Оптимальное пороговое значение, позволяющее различать норму и наличие стеатоза, составило 5,2 % (чувствительность 65 %, специфичность 93 %). Коэффициент корреляции методов PDFF с данными биопсии составил 0,66 (р ≤ 0,01).</p></sec><sec><title>Выводы</title><p>Выводы. Таким образом, магнитно-резонансная томография с оценкой протонной плотности фракции жира (PDFF) является высокоинформативным методом диагностики жировой болезни печени, в том числе у пациентов после трансплантации печени. Определение пороговых значений степеней жирового гепатоза требует проведения дальнейших исследований.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To evaluate the effectiveness of steatosis detection in liver transplant patients using magnetic resonance imaging with proton density quantification of liver fat fraction (PDFF).</p></sec><sec><title>Materials and methods</title><p>Materials and methods. А total of 113 liver transplant recipients (61 women and 52 men) aged 19 to 72 years were examined. All patients underwent magnetic resonance imaging (MRI) with proton density quantification of liver fat fraction. The obtained MRI (PDFF) data were compared with biopsy results. Calculation of the percentage of fatty infiltration of the liver based on right lobe liver biopsy data was used as a reference method.</p></sec><sec><title>Results</title><p>Results. According to biopsy data, steatosis was absent in 81 patients (72 %), while steatosis of varying severity was detected in 32 patients (28 %). The comparison revealed significant differences in the PDFF (p ≤ 0.001) between patients with and without steatosis. The optimal cutoff value for distinguishing between normal and the presence of steatosis was 5.2 % (sensitivity 93 %, specificity 65 %). The correlation coefficient between PDFF methods and biopsy data was 0.66 (p ≤ 0.01).</p></sec><sec><title>Conclusions</title><p>Conclusions. Thus, magnetic resonance imaging with proton density lipid fraction (PDFF) assessment is a highly informative method for diagnosing fatty liver disease, including in patients after liver transplantation.
